We collect health-related information you provide during consultations, including medical history, symptoms, diagnoses, prescriptions, lab results, imaging reports, treatment plans, and any other clinical data shared with our providers. This information is classified as Protected Health Information (PHI) under HIPAA and is handled with the highest level of security and confidentiality.
We collect payment card details, bank account information, insurance policy numbers, group numbers, and billing addresses to process payments and handle insurance claims. Payment card data is processed through PCI DSS-compliant payment processors and is not stored on our servers.
Automatically collected when you access our Services, including IP address, browser type and version, operating system, device identifiers, pages visited, time spent on pages, click patterns, referring URLs, and interaction data with features such as chat, booking forms, and telehealth tools.
We use essential cookies for authentication and security, functional cookies to remember your preferences, and analytics cookies (via Google Analytics and similar tools) to understand usage patterns. When you use our insurance services, we share necessary information with insurance carriers, clearinghouses, and billing partners to verify eligibility, submit claims, obtain pre-authorizations, and process reimbursements. This sharing is permitted under HIPAA's Treatment, Payment, and Healthcare Operations provisions.
With your explicit consent, we may send promotional emails about new services, wellness programs, seasonal health campaigns, partnerships, and exclusive offers. You can opt out of promotional communications at any time using the unsubscribe link in any email or through our Unsubscribe page, without affecting transactional communications.

Your health information is shared with licensed healthcare providers in our network who are directly involved in your care. Each provider is bound by HIPAA, professional confidentiality obligations, and our Business Associate Agreements. Providers access only the minimum necessary information required for your treatment.
We may disclose information when required by law, including in response to court orders, subpoenas, or government requests; to report suspected child abuse or neglect; to comply with public health reporting requirements; to health oversight agencies for audits and investigations; and to law enforcement in emergencies involving imminent harm.
In the event of a merger, acquisition, reorganization, bankruptcy, or sale of all or a portion of our assets, your information may be transferred as part of the transaction. We will notify you via email or prominent notice before your information becomes subject to a different privacy policy.
We implement industry-leading security measures: AES-256 encryption for data at rest, TLS 1.3 for data in transit, multi-factor authentication for all accounts, role-based access controls, continuous security monitoring, regular penetration testing by independent firms, SOC 2 Type II certified infrastructure, and mandatory HIPAA security training for all employees.
All data is encrypted using AES-256 (Federal Information Processing Standard 197) at rest. Our infrastructure runs on HIPAA-eligible cloud environments with dedicated VPCs, encrypted EBS volumes, IAM policies, security groups, and WAF (Web Application Firewall) protection. Database access is logged and auditable.
Access to PHI and sensitive data requires multi-factor authentication, minimum 12-character passwords with complexity requirements, session timeout after 15 minutes of inactivity, and IP allowlisting for administrative access. All access events are logged in an immutable audit trail.
All employees undergo background checks, sign confidentiality agreements, complete HIPAA training upon hiring and annually, and access only the data necessary for their role. Vendor personnel with data access are subject to equivalent requirements through Business Associate Agreements.
We retain your personal information for as long as your account is active or as needed to provide services. Health records are retained in accordance with state and federal medical record retention laws (typically 7–10 years after the last encounter). Payment records are retained for 7 years for tax and audit compliance. Marketing data is retained until you unsubscribe or request deletion.
You have the right to obtain a copy of the personal and health information we hold about you. You can request this through your account settings or by contacting privacy@fithealthresource.info. We will provide the information in a readable electronic format within 30 days, as required by HIPAA.
You may request correction of inaccurate or incomplete personal or health information. For health records, corrections are processed in coordination with your treating provider. You can submit correction requests through your patient portal or by contacting our Privacy Officer.
You may request deletion of your personal information, subject to legal retention requirements. Health records subject to HIPAA retention requirements cannot be deleted during the mandatory retention period but will be restricted from further use. Upon account deletion, marketing and non-essential data is permanently erased within 45 days.
You may request your personal data in a structured, commonly used, machine-readable format (such as JSON or CSV). Health records can be exported as a Continuity of Care Document (CCD) compatible with any EHR system, ensuring seamless transfer to another provider.
You may request restriction of certain uses of your PHI, particularly for disclosures to health plans for purposes of payment or healthcare operations when you have paid out of pocket in full. We will accommodate reasonable requests as required by HIPAA.
You can opt out of promotional emails, SMS, and push notifications at any time without affecting your access to healthcare services or transactional communications. Use the unsubscribe link, account preferences, or contact us directly. Opt-out requests are processed within 10 business days.
In addition to this Privacy Policy, we provide a separate HIPAA Notice of Privacy Practices (NPP) that details your rights under HIPAA, how we may use and disclose your PHI, and our legal duties. The NPP is available in your patient portal and provided at the time of first service.
Residents of States have additional rights under the State Consumer Privacy Act (CCPA) and the State Privacy Rights Act (CPRA), including the right to know, delete, correct, and opt out of sale/sharing. Residents of other states with privacy laws (Virginia, Colorado, Connecticut, etc.) have analogous rights under applicable state statutes.
Our Services are not directed to individuals under 13 years of age. We do not knowingly collect personal information from children under 13. If we discover such information has been collected, we will delete it immediately. For minors aged 13–17, a parent or legal guardian must provide consent for account creation and service use.
Your data is stored and processed primarily in the United States. In the event that data is transferred to or accessed from outside the U.S. (e.g., by cloud providers with international infrastructure), we ensure appropriate safeguards including Standard Contractual Clauses, adequacy determinations, and supplementary measures as required by applicable law.
In the event of a breach of unsecured PHI, we will notify affected individuals without unreasonable delay and no later than 60 days from discovery, as required by HIPAA. Notifications will be sent via email or first-class mail and will include a description of the breach, types of information involved, steps individuals should take, and our mitigation measures.
We may update this Privacy Policy periodically. Material changes will be communicated via email notification at least 30 days before taking effect, and the "Last updated" date will be revised. Continued use of our Services after changes take effect constitutes acceptance of the revised policy.

These Terms and Conditions ("Terms") govern your access to and use of the website, mobile applications, and healthcare services provided by Fit Health Resource. ("Fit Health Resource," "we," "us," or "our"). By accessing or using our Services, you acknowledge that you have read, understood, and agree to be bound by these Terms. If you do not agree, please discontinue use of our Services immediately.
By creating an account, booking an appointment, purchasing insurance, or using any Fit Health Resource service, you confirm that you are at least 18 years of age (or have parental consent), legally competent to enter into agreements, and accept these Terms in their entirety. These Terms constitute a legally binding agreement between you and Fit Health Resource.
Fit Health Resource provides a digital healthcare platform connecting patients with licensed healthcare providers for primary care, dermatology, dental care, haircare, weight loss management, fitness and wellness coaching, health insurance brokerage, and life insurance brokerage services. Specific services, availability, and pricing are listed on our platform and may vary by state and provider.
To use our Services, you must be a resident of the United States or its territories, at least 18 years of age (or 13 with parental/guardian consent for certain services), not prohibited from receiving healthcare services under applicable law, and capable of providing accurate and complete information. We reserve the right to refuse service to anyone who does not meet these criteria.
You must create an account to access full Services. You are responsible for maintaining the confidentiality of your login credentials, for all activities under your account, and for notifying us immediately of any unauthorized access. We are not liable for losses arising from your failure to secure your account. Passwords must meet minimum complexity requirements, and we strongly recommend enabling two-factor authentication.
You represent that all information you provide — including personal details, medical history, insurance information, and payment data — is accurate, complete, and current. Providing false or misleading information may result in inaccurate diagnoses, inappropriate treatment, insurance claim denials, and immediate account suspension.
Appointments are booked through our platform and subject to provider availability. Booking constitutes a request for services; confirmation is sent via email and/or SMS once the appointment is finalized. Appointment times are in the time zone of the selected provider's location unless otherwise specified during booking.
You may cancel or reschedule an appointment at no charge up to 24 hours before the scheduled time. Cancellations within 24 hours may incur a cancellation fee of up to $50, which will be clearly disclosed at the time of booking. Repeated late cancellations may result in restricted booking privileges.
If a provider cancels an appointment, we will notify you immediately and offer the earliest available alternative. You will not be charged for provider-initiated cancellations. If no suitable alternative is available, any pre-payment will be refunded in full within 5–7 business days.
If you fail to appear for a scheduled appointment without canceling, a no-show fee of up to $75 may be charged to your payment method on file. The specific fee amount is displayed during booking. Three or more no-shows within a rolling 90-day period may result in account suspension.
Rescheduling is free when done 24+ hours before the appointment. Rescheduling within 24 hours is treated as a late cancellation and may incur the cancellation fee. Rescheduled appointments are subject to provider availability and are not guaranteed at the original time slot.
Prices for each service are displayed on our platform before booking. Prices vary by service type, provider, consultation format (in-person vs. virtual), and geographic location. All prices are in US Dollars and include applicable taxes unless otherwise stated. We reserve the right to update pricing with 30 days' notice.
We accept major credit cards (Visa, Mastercard, American Express, Discover), debit cards, HSA/FSA cards, and digital wallets (Apple Pay, Google Pay). Payment is processed at the time of booking or at the conclusion of the service, as specified during the booking flow. All payments are processed through PCI DSS Level 1 compliant processors.
For services covered by insurance, we will bill your insurance carrier directly when you provide valid insurance information. You are responsible for any co-pays, co-insurance, deductibles, and non-covered charges. If insurance denies the claim, you are responsible for the full service fee. We are not responsible for insurance carrier decisions.
Payment is due at the time of service unless otherwise arranged. Overdue balances may incur a late fee of 1.5% per month (18% annually) on the outstanding balance. Failed payment attempts may incur a $25 processing fee. We reserve the right to suspend services and restrict appointment booking for accounts with balances overdue by 60+ days.
Refunds are issued for provider-initiated cancellations, service quality issues substantiated by our review team, and duplicate charges. Refunds for other reasons are evaluated on a case-by-case basis. Approved refunds are processed within 5–10 business days to the original payment method. Service fees for completed consultations are non-refundable.
Fit Health Resource acts as a licensed insurance broker for health insurance products. We are not the insurer. Our role is to present available plans, facilitate enrollment, and assist with claims. Policy terms, coverage, exclusions, and premiums are determined solely by the insurance carrier. We earn commissions from carriers, which do not affect your premium.
Similar to health insurance, Fit Health Resource acts as a licensed broker for life insurance products. We present options from multiple carriers, assist with applications and underwriting, and provide ongoing policy support. Coverage terms, riders, and premiums are set by the issuing carrier. We receive commissions that are disclosed to you before purchase.
To the maximum extent permitted by applicable law, Fit Health Resource's total aggregate liability arising from or related to these Terms or our Services shall not exceed the total fees you paid to us in the 12 months preceding the claim. We are not liable for any indirect, incidental, special, consequential, or punitive damages, including loss of profits, data, or health outcomes.
Our Services are provided "as is" and "as available" without warranties of any kind, express or implied, including merchantability, fitness for a particular purpose, non-infringement, or uninterrupted/error-free operation. We do not warrant that our platform will be available at all times, free from errors, or compatible with all devices or browsers.
Fit Health Resource is a platform that connects patients with healthcare providers; we do not directly provide medical diagnosis, treatment, or prescriptions. All clinical services are provided by independent, licensed practitioners who are solely responsible for the clinical care they deliver. We do not guarantee specific health outcomes from any service.
Our Services are not intended for medical emergencies. If you are experiencing a life-threatening emergency, call 911 immediately or go to the nearest emergency room. Do not rely on our platform for urgent medical care. We are not liable for any harm resulting from failure to seek emergency care when appropriate.
Telehealth consultations are subject to additional considerations: you must be physically located in a state where the provider is licensed; you must have a stable internet connection and compatible device; you must ensure privacy during the consultation; telehealth may not be appropriate for all conditions, and the provider may recommend in-person follow-up care.
All content, design, graphics, logos, software, and code on our platform are the property of Fit Health Resource or its licensors and are protected by US copyright, trademark, and other intellectual property laws. You may not reproduce, distribute, modify, or create derivative works without our prior written consent. The Fit Health Resource name and logo are registered trademarks.
You agree not to: use the Services for any unlawful purpose; provide false information; impersonate any person or entity; interfere with or disrupt the platform; attempt to gain unauthorized access to systems; harass, threaten, or abuse providers or staff; record consultations without explicit consent; or use automated tools to scrape or extract data.
Your use of our Services is also governed by our Privacy Policy, which is incorporated by reference into these Terms. By using our Services, you consent to the collection and use of your information as described in the Privacy Policy, including the handling of Protected Health Information under HIPAA.
You agree to indemnify, defend, and hold harmless Fit Health Resource, its officers, directors, employees, agents, affiliates, and service providers from and against any claims, liabilities, damages, losses, costs, and expenses (including reasonable attorney fees) arising from your use of the Services, your violation of these Terms, or your violation of any rights of a third party.
We are not liable for any failure or delay in performing our obligations due to circumstances beyond our reasonable control, including natural disasters, pandemics, government actions, wars, terrorism, labor disputes, power outages, internet disruptions, or failures of third-party services. We will make reasonable efforts to resume services as soon as practicable.
Disputes arising from these Terms shall first be addressed through our internal resolution process by contacting support@fithealthresource.info. Unresolved disputes shall be submitted to binding arbitration under the rules of the American Arbitration Association (AAA), conducted in USA. You waive the right to participate in class-action lawsuits or class-wide arbitration.
These Terms are governed by and construed in accordance with the laws of the USA, without regard to its conflict of law provisions. Any legal action not subject to arbitration must be filed in the state or federal courts located in USA, and you consent to the personal jurisdiction of those courts.
We reserve the right to modify these Terms at any time. Material changes will be communicated via email notification at least 30 days before the effective date. Non-material changes may be posted without advance notice. Continued use of our Services after changes take effect constitutes your acceptance of the revised Terms.
If any provision of these Terms is found to be unenforceable or invalid by a court of competent jurisdiction, that provision shall be limited or eliminated to the minimum extent necessary, and the remaining provisions shall remain in full force and effect. The invalidity of any provision shall not affect the validity of any other provision.
Our failure to enforce any right or provision of these Terms does not constitute a waiver of that right or provision. Any waiver must be in writing and signed by an authorized representative of Fit Health Resource to be effective. A waiver of one instance does not constitute a continuing waiver of future instances.
These Terms, together with the Privacy Policy, HIPAA Notice of Privacy Practices, and any applicable service-specific agreements, constitute the entire agreement between you and Fit Health Resource regarding the use of our Services and supersede all prior or contemporaneous communications, representations, or agreements, whether oral or written.

Unsubscribing removes your email address from our promotional mailing list. It means you will no longer receive marketing emails from Fit Health Resource, but your account and access to healthcare services remain fully active and unchanged.
Certain emails are essential to service delivery and cannot be unsubscribed. These include: appointment confirmations and reminders, telehealth session links, prescription notifications, lab and test results, billing receipts and invoices, insurance claim status updates, account security alerts (password resets, login alerts), and policy change notifications.
After unsubscribing, you will no longer receive: weekly health tips and wellness newsletters, seasonal health campaign emails, new service launch announcements, promotional offers and discount codes, partner and referral program emails, survey and feedback requests, event and webinar invitations, and personalized health recommendation emails.
Unsubscribe requests are processed within 10 business days. During this window, you may receive a small number of promotional emails that were already queued for delivery before your request was processed. We apologize for any inconvenience.
Your unsubscribe becomes effective on the date our system processes your request, not the date you submit it. A confirmation email will be sent once processing is complete, confirming the effective date and summarizing what has changed.
Upon successful processing, you will receive a single confirmation email (which is a transactional message) confirming that your email has been removed from promotional lists. This confirmation itself cannot be opted out of.
You can re-subscribe to promotional emails at any time by logging into your account and updating your email preferences, or by filling out the newsletter subscription form on our website. Re-subscription is immediate once confirmed.
If you prefer to reduce rather than eliminate promotional emails, you can manage preferences in your account settings. Options include: weekly digest only, monthly digest only, specific topics only (e.g., skincare tips but not dental tips), and new service announcements only.
You can manage all email preferences directly in your Fit Health Resource account under "Settings" then "Communication Preferences." This gives you granular control over which types of emails you receive without fully unsubscribing.
Unsubscribing from Fit Health Resource does not automatically unsubscribe you from communications sent by our marketing partners (e.g., insurance carriers, wellness brands). You must unsubscribe from each partner separately using the links in their emails.
Our platform may display third-party content or offers. Unsubscribing from Fit Health Resource emails does not affect communications from third parties you have separately engaged with through their own platforms or services.
Unsubscribing removes you from our active mailing list but does not delete your account data. Your personal and health information is retained in accordance with our Privacy Policy and applicable retention laws (HIPAA requires retention of health records for 7–10 years).
Unsubscribing from emails does not deactivate, suspend, or close your Fit Health Resource account. You retain full access to book appointments, use telehealth services, manage insurance, and access all platform features.
All healthcare services — including consultations, prescriptions, lab orders, insurance management, and wellness programs — remain fully accessible. Unsubscribing only affects email communications, not your ability to receive care.
Past newsletters and promotional content remain accessible through our website blog section even after unsubscribing. You can browse archived health tips and articles at any time without being on the mailing list.
Unsubscribing from email does not affect SMS text message notifications (appointment reminders, etc.). To opt out of SMS, reply STOP to any message or manage SMS preferences in your account settings under "Communication Preferences."
Mobile app push notifications are managed separately from email subscriptions. To disable push notifications, go to your device Settings then Notifications then Fit Health Resource, or manage them in the app under Settings.
Notifications displayed within the Fit Health Resource app (e.g., new messages from providers, appointment updates) are not affected by email unsubscribe and must be managed separately in the app's notification settings.
Messages sent directly between you and your healthcare provider through our platform are not marketing communications and are not affected by unsubscribing. These messages are part of your clinical care and are retained as part of your medical record.
Appointment reminder emails and SMS messages are considered transactional and will continue after unsubscribing. These are essential to ensuring you do not miss scheduled healthcare appointments. To disable, adjust preferences in account settings.
Emails related to your insurance policies — including enrollment confirmations, premium due notices, claim status updates, and policy renewal reminders — are transactional and will continue. These are necessary for maintaining your insurance coverage.
Security-related communications — including data breach notifications, suspicious activity alerts, password reset emails, and critical system updates — cannot be unsubscribed. These are sent for your protection and comply with legal obligations.
Post-appointment satisfaction surveys and feedback requests will stop after unsubscribing. However, you can always provide feedback directly through your account dashboard or by contacting support@fithealthresource.info at any time.
If you experience issues with unsubscribing (e.g., still receiving emails after 10 business days), contact our support team at support@fithealthresource.info or call +1 347-204-7145. We will investigate and resolve the issue promptly.
If you believe your unsubscribe request was not processed correctly or your data rights have been violated, you may file a formal grievance with our Privacy Officer at privacy@fithealthresource.info. We will acknowledge your grievance within 5 business days and resolve it within 30 days.
Our unsubscribe process complies with the CAN-SPAM Act (15 USC 7701), HIPAA communication requirements, CCPA/CPRA opt-out rights, and state-specific marketing regulations. We honor all opt-out requests regardless of the method used (link, form, email, phone).
Unsubscribing does not waive any of your rights under our Privacy Policy, HIPAA, CCPA, or other applicable laws. You retain the right to access, correct, delete, and port your personal and health information at any time, regardless of your subscription status.
To protect against unauthorized unsubscribes, we may send a verification email requiring you to confirm the request by clicking a link. This ensures that a third party cannot remove you from our mailing list without your knowledge.
Rather than a binary subscribe/unsubscribe, we encourage you to use our preference center for granular control. Choose exactly which categories of emails you want: health tips, promotions, service updates, partner offers, surveys, and seasonal campaigns — each independently toggleable.
Limited-time promotional offers, discount codes, and flash sale notifications will cease after unsubscribing. Please note that offers referenced in promotional emails have expiration dates and may no longer be available if you re-subscribe at a later date.
Seasonal health campaigns (e.g., flu prevention, summer wellness, back-to-school checkups) and special awareness month communications will stop after unsubscribing. These campaigns typically run during specific months and will not be sent again once you're off the list.
After unsubscribing, your email address is moved from the active marketing list to a suppression list. It is not deleted from our database (to prevent accidental re-addition) but is flagged as "do not contact — promotional." This suppression status is respected across all our marketing systems and tools.
